Sports medicine claims hinge on injections and visits

A sports medicine visit often pairs an evaluation with a joint injection, ultrasound guidance, and a brace. Each piece has its own billing rule, and the wrong combination gets cut. This page covers nonoperative sports medicine; surgical cases belong on the orthopedic surgery page.

Where sports medicine claims get cut

Most problems come from how injections, guidance, and visits are combined on one claim.

One joint, one unit

Arthrocentesis codes 20600-20611 are billed per joint, including its surrounding bursae. Injecting a shoulder joint and a bursa of the same shoulder is still one unit.

Guidance already in the code

Codes 20604, 20606, and 20611 describe injections done with ultrasound guidance. When a code includes guidance, a separate ultrasound guidance code is not reported.

Visits on injection day

Minor procedures carry 0- or 10-day global periods, so a same-day visit is generally part of the procedure. A significant, separately identifiable evaluation can be billed with modifier 25 when the note supports it.

Splitting the work with your practice

Sports medicine practices often share a building with athletic trainers, physical therapists, and surgeons.

What your practice keeps

Physicians document the exam, the joint injected, the guidance used, and any brace ordered. They approve every code Taiga drafts.

What Taiga takes on

Taiga checks eligibility, scrubs claims against payer rules, submits them, and works denials and appeals down to small balances. It also handles patient statements and balance follow-up.

What needs a decision

Decide who owns bracing, which falls under Medicare's orthotics benefit and its supplier rules, and how sports physicals and team coverage are priced.

Questions for a sports medicine fit call

Bring de-identified notes for an injection visit, a concussion follow-up, and a brace fitting.

What is your injection mix?

List the joints you inject most and how often you use ultrasound guidance.

Do you dispense braces?

Explain whether braces are billed by your practice or by a separate supplier.

Who else shares the care?

Describe how patients move between your clinic, therapists, athletic trainers, and surgeons.

For nonoperative sports medicine practices

Useful for an independent primary care sports medicine practice that wants injection claims coded correctly and every denial followed up.

Where this page stops

Sports medicine is not on Taiga's published specialty list, so bring your service mix and Taiga will tell you straight whether it fits. Operative sports cases belong on the orthopedic surgery page, a specialty Taiga lists as supported.

Questions about sports medicine billing

Can we bill 76942 with 20611?

No. Code 20611 already includes ultrasound guidance, so NCCI policy bars a separate guidance code for the same injection.

How many units for a shoulder joint and bursa injection?

One. NCCI counts a joint and its surrounding bursae as a single unit of arthrocentesis.

Can we bill a visit and an injection on the same day?

Only when the visit is significant and separately identifiable from the decision to inject, reported with modifier 25. A routine check before the injection is part of the procedure.

How is this different from your orthopedics page?

This page covers clinic-based care, injections, and bracing. Operative cases are on the orthopedic surgery page, so bring both if your practice does both.

Bring your clinic and injection mix

Show Taiga a typical week of visits, injections, and bracing. If it fits, onboarding takes about two weeks to first claim.
