Allergy billing counts tests, doses, and shots

Allergy practices bill in units: each skin test, each antigen dose prepared, each injection visit. Medicare has its own rules for counting antigen doses from a multidose vial. Small counting errors repeat across hundreds of patients.

Where allergy claims are counted

Testing, antigen preparation, and injections each carry their own unit rules.

Allergy testing

Medicare fee amounts for allergy testing codes 95004 through 95078 are set per test, so the number of tests goes on the claim. Twenty-five prick tests bill as 95004 with 25 units.

Antigen preparation

Medicare treats 95165 as preparing 1 cc doses from a multidose vial. A practice may bill up to 10 doses per vial, and fewer when the vial holds less than 10 cc.

Injections and immunology care

The injection itself is billed separately from antigen preparation. Immunology work, such as biologics for asthma or immune deficiency care, adds drug and administration claims to ordinary visits.

Line up the vial log with the claim

Allergy billing depends on mixing records and injection logs as much as on visit notes.

What the practice keeps

Your allergists document test results, extract prescriptions, and reactions. Staff keep mixing and injection logs, and physicians approve codes before submission.

What Taiga would take on

With the fit confirmed, Taiga would draft codes from notes and logs, check that units match tests and doses, verify eligibility, and work every denial and appeal.

Where a decision is needed

Shots given from your vials at another office raise questions about who bills the preparation and who bills the injection. Settle that before claims move.

Questions for an allergy fit call

Bring a de-identified testing note, a mixing log, and a few recent denials. You will not need patient identifiers to book.

How do you prepare extracts?

Explain vial sizes, who mixes, and how doses per vial are recorded.

What testing do you run?

List prick, intradermal, patch, and food challenge testing, with typical test counts per visit.

Which immunology services do you bill?

Describe biologic treatment, immune deficiency care, and any infusions done in office.

For allergy and immunology practices

Worth a conversation if your practice bills its own claims and wants testing and immunotherapy units checked against the record every time. Your EHR stays as it is.

Compounding standards are a clinical matter

This page is about billing. Compounding standards for allergen extracts are a clinical and pharmacy question, and ENT practices that offer allergy care are covered on the ENT page.

Questions about allergy and immunology billing

Does Taiga work with allergists?

Allergy and immunology are not on Taiga's public specialty list. Bring your service mix to a call and you will get a plain answer on fit.

How many units of 95165 can we bill per vial?

Medicare allows up to 10 doses per multidose vial, counted as 1 cc aliquots. If the vial holds less than 10 cc, bill the number of 1 cc doses it can provide.

How do we bill skin testing?

Allergy testing codes are priced per test, so report the code with the number of tests as units.

Is the injection included in antigen preparation?

No. 95165 covers preparation only, and the injection is billed separately.

Bring your testing and shot volume
