Medicare's telehealth flexibilities now end December 31, 2027

Under the CMS telehealth FAQ updated in October 2026, Medicare patients can receive telehealth anywhere in the U.S. through December 31, 2027. Starting in 2028, most non-behavioral visits return to rural and facility limits, so virtual-first practices need a plan.

Rules a virtual practice bills against

These come from the CMS Telehealth FAQ dated October 2026 and the CY 2026 Medicare telehealth services list.

Place of service

Use POS 10 when the patient is at home and POS 02 for telehealth in other locations. Since 2024, Medicare has paid home telehealth at the non-facility rate.

The 2028 cutoff

After December 31, 2027, most non-behavioral telehealth again requires the patient to be in a medical facility in a rural area. Physical therapists, occupational therapists, speech-language pathologists, and audiologists also lose telehealth billing then.

Behavioral health rules

Behavioral telehealth has no geographic limits and allows audio-only. From 2028, home mental health telehealth needs periodic in-person visits: every 12 months for patients who started earlier, plus one within 6 months before the first telehealth service for new patients.

Sort what the platform does from what billing does

Video visits produce notes like any other visit. The billing questions sit in location, modality, and payer.

What the practice keeps

Clinicians document the visit, the patient's location, and whether it was audio-video or audio-only. The physician approves codes before submission.

What Taiga takes on

Taiga drafts codes from notes, checks eligibility, scrubs claims against each payer's telehealth rules, submits them, and works every denial and appeal.

Where a decision is needed

Multi-state licensure, commercial plan telehealth policies, and how you will serve Medicare patients after 2027 need an owner on your side.

Questions for a telehealth fit call

Bring de-identified examples of a paid video visit, an audio-only visit, and a denial. Do not put patient information in the booking form.

What share of visits are virtual?

Estimate video, audio-only, and in-person shares by payer.

Which services do you deliver?

List specialties and visit types, since behavioral and non-behavioral telehealth follow different Medicare rules.

Where are your clinicians and patients?

Note the states you serve and whether any clinicians work only from a home address.

For practices that see most patients on screen

Useful to a virtual-first or hybrid practice that wants telehealth claims coded for location and modality, with every denial followed up.

Telehealth law keeps moving

Congress and CMS have changed these dates several times. This page reflects the CMS FAQ updated in October 2026 and is not legal or coverage advice.

Questions about telehealth billing

Can Medicare patients still have telehealth visits from home?

Yes. CMS says beneficiaries can receive Medicare telehealth anywhere in the U.S. and territories through December 31, 2027. Behavioral health telehealth at home continues after that date.

Which place of service code do we use for telehealth?

POS 10 when the patient is at home and POS 02 when they are somewhere else. Home telehealth is paid at the non-facility rate.

Is audio-only telehealth allowed?

Through December 31, 2027, Medicare patients can receive audio-only telehealth at home. From 2028 it is limited to behavioral health when the clinician can do video but the patient can't or won't.

Do clinicians working from home need to enroll their home address?

Not if they have a physical practice location; they can bill from there. Virtual-only clinicians whose only location is home must enroll it, marked for administrative or telehealth use.
