Home and residence visits
Visits in a patient's home or assisted living use the home or residence E/M codes. Starting January 1, 2026, Medicare allows the G2211 add-on with those codes, not only with office visits.
Older patients are seen in the clinic, at home, in assisted living, and in nursing facilities. Many visits involve caregivers, cognitive concerns, and planning conversations that Medicare pays as separate services with their own documentation rules. Bring your service mix and Taiga will tell you straight whether it fits.

CPT 99483 covers a detailed cognitive assessment and a written care plan. An independent historian such as a spouse or caregiver must be present, and the service can be billed on the same day as an Annual Wellness Visit with modifier 25.
CPT 99497 and 99498 are time-based. The record must show the visit was voluntary, who was present, and the minutes spent. A discussion of 15 minutes or less is billed as another E/M service instead.
Time-based and caregiver-dependent services need documentation that billing can actually use.

Geriatric medicine is not among Taiga's named specialties. Nursing facility billing arrangements, hospice coordination, and similar edge cases are reviewed claim type by claim type.
Yes, under Medicare. Add modifier 25 when both happen in the same visit, and document the independent historian who was present.
Not with 99497. Medicare says a discussion of 15 minutes or less should be billed as another E/M service, such as an office visit.
Starting January 1, 2026, Medicare allows G2211 with the home or residence E/M visit codes as well as office and outpatient visits.
