Midwifery billing changes with the 2027 maternity codes

On January 1, 2027, global maternity codes give way to visit-level prenatal and postpartum billing plus new labor management and delivery codes. For midwives, labor management is the part to watch, because long labors can now be reported by calendar day. Credentials and state Medicaid rules still decide who can bill what.

What shapes a midwifery claim

Who the midwife is, where the birth happens, and which payer covers it matter as much as the code.

The 2027 maternity code set

Antepartum and postpartum visits move to E/M codes billed as they happen, and labor gets its own management codes, 59080 through 59083, reported with the delivery code. Practices used to one claim after the birth will need to submit claims throughout care.

Credential and payer rules

Medicare pays certified nurse-midwives at 100 percent of the physician fee schedule. Every state Medicaid program pays CNMs, but coverage for midwives without a nursing degree, and the rate for both, varies by state and plan.

Birth setting

Hospital, freestanding birth center, and home births use different place of service codes, and some states limit where certain midwives can be paid. Where a payer covers birth center services, the center's facility claim is separate from the midwife's professional claim.

Match billing to the course of care

Visit-level billing means claims follow the care as it happens, not after the birth.

The practice keeps

Midwives document each prenatal visit, labor day, delivery, and postpartum visit and approve the codes Taiga drafts. Transfer-of-care notes stay with the clinical team.

Taiga takes on

Prenatal, postpartum, and gynecologic visit claims as they happen, with eligibility checks, scrubbing, submission, and every denial and appeal. Patient statements and follow-up come with it.

Decide together

Transfers to hospital care during labor, birth center facility claims, and pregnancies that began under global billing need a clear owner. Agree on the split before the first 2027 deliveries.

Questions for a midwifery fit call

Bring de-identified examples and your payers' maternity policies. Keep patient details out of the booking form.

Which credentials bill?

List CNMs, certified midwives, and licensed or professional midwives, their NPIs, and which payers have credentialed each one.

Where do births happen?

Break out hospital, birth center, and home births, and who sends records when a laboring patient transfers.

Which payers still expect one claim after birth?

Note the plans that already accept visit-level billing and those still asking for a single maternity claim, so pregnancies spanning the change are handled correctly.

Worth a call for CNM practices and birth centers

Useful to an independent midwifery practice or birth center preparing for visit-level maternity billing that wants claims, denials, and appeals handled in step with care, inside its current EHR.

State rules decide who can bill

Licensure, Medicaid enrollment, and covered birth settings differ by state and by plan. This page does not determine whether a midwife or setting is payable, or how any visit should be coded.

Questions about midwifery billing

What changes for midwives on January 1, 2027?

Global maternity codes are deleted. Prenatal and postpartum visits are billed as E/M encounters, and labor management and delivery have separate codes.

Does Medicare pay certified nurse-midwives the same as physicians?

Yes. Medicare Part B pays CNM services at 100 percent of the physician fee schedule amount for the same service.

Does Medicaid pay midwives without a nursing degree?

In some states. Every state pays certified nurse-midwives, but coverage for other midwives depends on state licensure and Medicaid policy.

Is routine postpartum care on the day of birth billed separately?

Not under the 2027 codes. Routine postpartum care on the delivery day is included in the delivery code, and later postpartum visits are billed as E/M encounters.
