Global periods decide what general surgeons get paid

Most general surgery revenue comes from 10- and 90-day global packages, where follow-up is prepaid and extra work needs a documented reason. Medicare now expects modifier 54 whenever the surgeon does not plan to provide post-op care, even without a formal transfer. Hernia and mesh bundling rules add another layer.

Where general surgery claims slip

These rules come from Medicare's global surgery and correct coding policies.

Transfer of care, formal or not

Since 2025, a surgeon who does not expect to see the patient after a 90-day global procedure bills it with modifier 54, written transfer agreement or not. A clinician outside the surgeon's group who provides post-op visits can add HCPCS G0559 once in that period.

Hernia repair and mesh

Most hernia repair codes include mesh placement, so a separate mesh code is generally not reported. A hernia repaired at the incision site of another open abdominal procedure is not billed separately.

Incidental work in the same field

Removing a normal appendix during another abdominal operation, or a panniculectomy at the incision for a hernia repair, is not separately reportable. A medically necessary appendectomy can be.

Splitting the work with your practice

General surgeons often cover call at more than one hospital, which scatters notes and charges.

What your practice keeps

Surgeons document operative findings, hernia type and defect size, and who will provide care after surgery. They approve codes before submission.

What Taiga takes on

Taiga drafts codes from operative and clinic notes, verifies eligibility, scrubs claims against payer rules, submits them, and appeals every denial. It also sends patient statements and follows up on balances.

What needs a decision

Call coverage arrangements, assistant-at-surgery billing, and any endoscopy or bariatric program need a clear scope decision.

Questions for a general surgery fit call

Bring de-identified examples of an elective hernia repair, an emergency case from call, and a post-op visit.

How are call cases captured?

Explain how charges from overnight and weekend hospital cases reach billing.

Who sees patients after surgery?

Describe when post-op care moves to another clinician or practice, and how you record it.

What denials repeat?

List the denial reasons you see most, such as bundling edits or modifier problems.

For independent general surgery practices

Useful for a physician-owned general surgery group that wants global periods tracked, bundling edits handled, and every denial pursued.

Where this page stops

General surgery is not on Taiga's published specialty list, so bring your service mix and Taiga will tell you straight whether it fits. Facility claims for a surgery center are covered on a separate page.

Questions about general surgery billing

When do we add modifier 54 now?

For 90-day globals, Medicare expects modifier 54 whenever the surgeon does not plan to provide the post-op care, with or without a formal transfer agreement. If the surgeon will provide all of it, the global code goes out without that modifier.

Can we bill mesh separately with a hernia repair?

Generally no. Most hernia repair codes include mesh or other prosthesis, so NCCI policy bars a separate mesh code unless CPT instructions allow one.

Did the 2026 Medicare fee schedule change surgical payment?

Yes. CMS applied a -2.5% efficiency adjustment to select non-time-based services, including surgical procedures, in the 2026 physician fee schedule.

Does Taiga require us to switch EHRs?

No. Taiga works with your current EHR and drafts codes from operative notes for surgeon sign-off.
